Team Leader – Support at Home

The Rural City of Wangaratta is looking for a passionate Team Leader to guide our Support at Home service and make a real impact in the community. This role is all about people: leading a skilled team, shaping high-quality services and keeping operations running smoothly in a rapidly evolving aged care environment.
As a key leader, you will oversee the day-to-day operations and strategic direction of our Packaged Care team, including case managers, support staff and administration. You’ll ensure services are delivered with empathy, efficiency and in line with the latest aged care standards and reforms. What You’ll Lead & Deliver
- Inspire, coach and support a multidisciplinary team through recruitment, onboarding, performance development and day-to-day leadership
- Coordinate the delivery of tailored, client-directed services that reflect individual needs, goals and personal choices
- Provide expert advice on aged care reforms, sector trends, service gaps and compliance requirements
- Monitor client budgets, service outcomes and quality indicators to ensure sustainable, high-quality care
- Contribute to annual budgets, business planning and reporting aligned with funding and legislative frameworks
- Maintain oversight of client data, compliance reporting, risk management, SIRS and mandatory reporting
- Drive continuous improvement, champion innovation and support new ways of working
- Build strong relationships across community networks, providers and partner agencies
What You’ll Bring
- Tertiary qualifications in aged care, health, community services or a related human services field
- Demonstrated experience leading aged or disability services, including staff supervision and service planning
- Strong knowledge of the Aged Care Act 2024, aged care reforms, care standards and wellness/reablement approaches
- Confident, people-focused leadership with the ability to motivate, coach and unify a diverse team
- Experience managing client budgets, service data and using insights to drive improvement
- Excellent communication, negotiation and stakeholder engagement skills
- Solid administrative and digital capability, including Microsoft Office and client management systems
- A valid driver’s licence and clear police check, with no listing on the Aged Care Banning Orders Register
