Clinical Team Leader – Aged Care Assessment
Wangaratta, VIC | Temporary Full Time until March 2027 | $97,906 per annum + super

If you are a clinical leader who wants your work to have genuine impact, this role offers the opportunity to guide a dedicated Assessment team and help older people in the Rural City of Wangaratta access the right support at the right time. You will shape how aged care services are delivered and play a key role in strengthening outcomes for our community.
What you will be doing
You will lead the daily operations of the Aged Care Assessment team, triage new referrals and provide clinical oversight to ensure assessments and support plans are completed with accuracy and care. You will support staff development, improve systems and processes, and contribute to service planning and continuous improvement. You will also build strong relationships across the aged care and community sectors, identify emerging needs and help ensure our services remain responsive, high quality and aligned with community expectations.
Key responsibilities
- Manage daily workflow, triage referrals and allocate assessments
- Provide clinical oversight and support to Assessment Officers and support staff
- Maintain strong knowledge of My Aged Care, service providers and support pathways
- Guide staff in completing assessments, support plans and reviews
- Participate in networks, forums and stakeholder engagement activities
- Identify service gaps and emerging needs and provide advice to leadership
- Contribute to staff training, quality improvement and business planning
- Support recruitment, onboarding, performance management and professional development
- Assist with budget monitoring and reporting
- Ensure compliance with quality frameworks and operational requirements
